Egghead 5 is getting on for a year old now, so it's probably time to let the cat out of the bag regarding hidden screens. So this is where they are, and how to access them. They are best accessed in the puzzle game rather than the collect-em-up option as a couple of screens cannot be reached at all in the latter version. Also, the hidden screens contain bonus extra lives in the puzzle game, but not in the collect-em-up.
Greece.
This screen is simplest of all to find, and lots of players have already done so. At the start of the game, and whenever Egghead's yacht is in Greece, there is an extra concealed screen on the sea bed. Head towards Holed, and go to the bottom right of the screen. You can then jump up and right through the sand, to access Atlantis. If you can find your way past the starfish and conveyor belt, you should be able to pick up an extra life here.
Egypt.
Pavero may have been the first to find these hidden screens, but he's not the only one. Enter the pyramid, and head down and right to the bottom. In Foot of the Pyramid, go down the bottom ladder, then move right and you will drop into some quicksand. While still in the sand, move left to It's Dark in Here, and keep walking left as far as you can. You will then drop into Secret Tunnel, which leads to the west side of the pyramid, a few screens to the left. Climb the west side to the summit, and you will discover an extra life.
Spain.
Firstly, head towards the Abandoned Villa. If you look at the bottom of the screen, you will see a lobster patrolling an area that the player would not need to visit, at least at first glance, as it appears to lead nowhere. However, at the bottom left the sand turns to quicksand, and you can drop through to the Wine Cellar, where an extra life is located.
The next area isn't really a hidden screen, it's more a part of the plot - it is one of the puzzles you need to solve to find one of Egghead's missing friends. Deep underground, directly underneath Cardona Salt Mine, is the Unfinished Railway. It seems to me that the one thing you might need to complete an underground railway would be some sort of digging implement. Taking such a tool onto this screen will create an extra exit to the right, leading to an underground river and Mornington Crescent where you will find something of interest...
France.
This is the one I figured would give people the most trouble of all. From Sea Shore, head left until you reach Dans La Ville. Head to the exit at the bottom left of the screen, but instead of going down towards Etre Ou N'etre Pas, stand on top of the very bottom ladder, and jump left. There is a hidden door in the wall, and Egghead will fall through into a new Screen, Moulin Ruse. Alas, there are no naked dancing ladies, but there is an extra life to be had at the back of the theatre.
And that should be pretty much it, unless there are those who still haven't made it past Mr. Fogarty to discover the ZX Shed offices...?

But I still have some unanswered questions:
1) I would like to add names of all friends, but I don't know them.
2) You talked about 27 items, but I found 26 items only

3) For many items I don't have any usage - horseshoe, pear, umbrella, keys, sack, oranges, mushroom, spanner, pool 8-ball and bomb.
4) How to finish puzzle version of the game? I brought back all five friends and all items to the yacht but nothing happened.

1. Only Shelley has a name at the moment. Maybe I'll come up with some names if I do a sixth game.
2. The 27th object has to be created from one of the others.
3. One of these items has a use, the rest are red herrings.
4. You need something to help swallow the aspirins. Some juice, perhaps?
