Accidentally pressing the "nuke planet" button when you meant to press something else in Captain Blood was/is always a good one. They ought to have had a nice little black and white picture of Blood himself doing a Frank Spencer-ish finger to the mouth "oops" face when it happened.
Not Speccy related but I've been playing Civilization Revolution a lot lately and been skipping a lot of text. One of the downsides to this is it's very easy to agree to something in negotiations that you didn't mean to.
