Sounds like your system has a Z80 with a duff M1 line. If thats the case just replace the Z80 with a new one.
But try the Divide+ on anther spectrum first just to be on the safe side. -

Is there a way to tell if I have one of the bad z80 without the M1 line, does the CPU look differently, has special marking, serial number, etc.?
Is there any software test to test for it?

It would be nice to have in DivIDE some kind of CPU test for M1 line, since this is main reason why diviIDE fails.
In general DivIDE should have possibility to boot with test ROM (the best - content loaded from card) without auto-mapper. -
