The PlusD doubles the length of the 'steprate' specially when resetting the head to TRK0. Something might be critical there..... You could try to poke STPRAT (#2003) with a higher value and see if it makes any difference.

i had this issue a few times on my plusd but not had it for a while now
not sure if it was the fact i was using some old dodgy disks or the fact i have changed over to the unidos chip that stopped it but not had any problems lately

There where the many multiface versions, several variations of these (I think ZX Guraranteed did one as well), the PlusD (which with Plus D Hacker was ideal for this), and several others....
