it's specemu. (and possibly many others eg. unreal)
just press F4 and while in full screen use pulldown menu to exit. it will start in full screen next time you start specemu. -

Except I have a PCI X16 ATI RadeonX1650 512MB DDR2.
It only does it with Direct Draw turned off in the options of SpecEmu. -
