To be sure that the 5v doesn't hurt pin 16 on your tv, you usually only need a resistor at ca 1K in series between the computer and pin 16. All tv's have internal circiuts to prevent this voltage from getting too high anyway. -

Well, you can use the modulator case, aluminium heatsink or centre pin of the regulator as a ground (negative) connection.
